Achebe Hope Foundation's Community Health Outreach Programme works with qualified healthcare professionals and partners to bring health education, appropriate screenings, preventive health awareness and pathways to care closer to people who may otherwise struggle to access them.
Sometimes bringing healthcare closer is the first step towards helping someone seek care sooner.
Why People Delay Care
For families already under financial pressure, seeking healthcare can involve more than the cost of treatment. There may be transportation costs, time away from work, distance to a health facility and competing household expenses.
And when someone does not yet feel seriously unwell, seeking preventive care can easily be postponed. The difficulty is that some health conditions may develop without obvious early symptoms.
Access to reliable information matters too. Communities need clear, trustworthy health education that helps people understand prevention, healthy practices and when professional medical attention should be sought.
That is why outreach matters.
How Our Outreach Works
Bring information and appropriate services closer to the community.
Where appropriate, health outreaches are organised with community leaders, healthcare professionals and partner organisations in locations accessible to the people we hope to reach.
Depending on the programme and available clinical partners, activities may include health education, basic health checks, blood pressure or blood glucose screening, maternal and child health awareness, hygiene promotion and appropriate referrals. Clinical activities are undertaken by suitably qualified healthcare professionals.
And where screening identifies a potential concern, the objective should not end with telling someone there may be a problem. Where possible, we want outreach to become a bridge towards appropriate continuing care.

Who We Hope to Reach
Our programme is designed particularly for underserved communities, vulnerable households, older people, mothers, young children, and people for whom distance or financial pressure makes preventive healthcare more difficult to access.
We are especially interested in reaching people before a health concern becomes a crisis. Because prevention, awareness and earlier access to appropriate care can matter enormously.
Connected Care
Health rarely exists separately from the other challenges facing a household.
A mother may also need support through Maternal & Child Health. A family experiencing food insecurity may require Food & Humanitarian Relief. Children whose health affects their education may connect with Education & Learning, while relationships built through Community Development can help us understand wider community needs.
This is why our programmes are designed to connect. We want to see the person behind the health need — and the wider circumstances affecting their wellbeing.
This programme depends on professional expertise as much as financial support.
Doctors, nurses, pharmacists, laboratory professionals, community health workers, healthcare organisations and responsible health-sector partners can contribute expertise, time, equipment, appropriate supplies and programme support.
Individuals and organisations can also help fund outreach activities and the practical resources required to deliver them.
